Change of Member Information

NEW MEMBERS
Required for all prospective members joining the Auxiliary. If qualified in Direct Operations (i.e. Crew, Watchstanding, Coxswain...) they will be required to complete the direct operations forms. Please complete the following forms:

  • New Member Checklist.
  • Enrollment Application: ANSC Form 7001.
  • Prospective Member Interview Record: ANSC Form 7036.
  • New Member Course Study Guide.
  • New Member Reference Guide.
  • New Member Course Exam.
  • New Member Test Answer Sheet: CG-4886a ANSC Form 7010.
  • Photo ID Card Application.
  • Two (2) Fingerprint Cards: Form FD-258.
  • Verification of US Citizenship Form.
  • Authorization for Release of Information: Form SF-85.
  • Special Agreement Check: Form OFI-86c.
  • IDENTIFICATION CARDS
    Please complete the following application and bring with you when you have your photograph taken for the Auxiliary Photo ID. The information will be used to complete your Identification. This application and new photographs must be completed for ALL those who have old ID cards.

  • Photo Session
  • Photo ID Card Application.

    OPERATIONAL SUPPORT
    Required for all Auxiliarists. It is also the minimum level of clearance for any Auxiliarist that wishes to remain an official member of the USCGAUX. (RETIRED status members are NOT required to undergo a security investigation). Please complete the following forms:

  • Operational Support Checklist
  • Two (2) Fingerprint Cards: Form FD-258.
  • Verification of US Citizenship Form
  • Authorization for Release of Information: Form SF-85
  • Special Agreement Check: Form OFI-86c
  • DIRECT OPERATIONS
    Required for Auxiliarists involved with Aviation, National Officer, and District Commodores. Please complete the following forms:

  • Direction Operations Checklist
  • Two (2) Fingerprint Cards: Form FD-258.
  • Verification of US Citizenship Form
  • SF-86 (Questionaire for National Security Position)
  • SF-86A (Continuation Sheet(Only If Necessary))
  • DHS 11000-9 (Disclosure and Authorization Pertaining to Consumer Reports Pursuant to the Fair Credit Reporting Act)
